Position: Case Manager | Resident Advisor – Tillamook HOPE Center
Pay Range:
Pay Rate 1 - Case Manager: $20.
00 -$22.
00 per hour; Pay Rate 2 - Resident Advisor: $17.
00 - $18.
00 per hour.

Based in: Tillamook, OR Reports to: Facility Director & Lead Case Manager Case Manager: Overall Purpose of this Role: Every participant in our reentry program is assigned a case manager to provide support.
It is the responsibility of the case manager to ensure that assigned participants are meeting program requirements and setting and achieving goals.
Case managers are also responsible for identifying appropriate resources to support participant success.
The ultimate goal of the program is to transition participants into long-term housing.
Work Schedule: 20 hours per week; Must be able to work a flexible schedule with the possibility of some occasional early morning, evening, and weekend work.
Key Accountabilities: Case Management: Assess each participant’s needs and obtain knowledge of each person’s history through interviewing, records review, and consultation.
Schedule and conduct monthly meetings with assigned participants to create and update individual reentry plans (IRPs) with new goals, as well as assess the status of prior goals to determine achievement and/or other strategies for completion.
Help participants achieve and maintain program compliance.
Assist participants with navigating external resources to achieve their goals and/or maintain program eligibility, such as: scheduling appointments with the DMV, applying for SNAP benefits, referring them to affordable health care and/or housing resources, finding and scheduling with medical providers, job searches, etc.
Collaborate with other professionals involved in the participant’s success, such as: sponsors, doctors, lawyers, DHS workers, counselors, peer support specialists, family members, parole officers, etc.
Maintain confidentiality of each person involved in our reentry program.
Crisis Intervention: Consult with participants experiencing a crisis by de-escalating situations using trauma-informed techniques.
Identify and facilitate connection with appropriate resources in-the-moment.
Other Duties: Build a resource network to support participants in a variety of areas, such as: medical, mental health, DUI classes, social groups, financial counseling/coaching, legal support, etc.
Develop relationships with local property managers, landlords, housing authorities, and developers of affordable housing.
Maintain current information on housing opportunities, community resources and applicable laws.
Provide education and resources to participants on how to secure and maintain permanent housing.
Coordinate the application and payment of rental assistance for program participants.
Resident Advisor: Work Schedule: 20 hours per week; Must be able to work a flexible schedule with the possibility of some occasional early morning, evening, and weekend work.
Position Summary Supervise shelter activities to ensure the safety of participants.
Responsible for shelter intakes, reentry interviews, and after-hours referrals.
Manage timely bunk assignments, dorm inspections, and property storage.
Demonstrated ability to work independently.
Ability to appropriately assess a situation and take corrective action as necessary.
Written and verbal communication skills.
Good computer skills and phone etiquette.
Responsible for maintaining accurate logs, shift reports, and participant records, both manually and using a web-based program.
Perform urinalysis and breathalyzer testing of participants as needed.
Attend agency and program staff meetings.
Facilitate problem-solving and provide crisis intervention services to participants as necessary.
The right individual for this position has a passion for helping others, is nonjudgmental, and is able to deal with difficult situations in positive and constructive ways.
Experience working with homeless/low-income population helpful.
Understanding of the issues surrounding homelessness, chemical dependency, domestic violence, and sexual assault.

Frequently required to sit/stand at a computer or in a stationary position.
Frequently operates a computer and other office productivity equipment, such as copy machine, printer, and phone.
Frequently required to move about within the facility for meetings, office equipment, and daily operations.
Education, Skill, Experience and/or Training Requirements: Basic technology skills, such as using Microsoft programs (word processing and spreadsheets), using a computer, inputting information into software, using a smart phone, and sending emails.
High School Diploma or Equivalent Excellent interpersonal, oral, and written communication skills.
Minimum of one year of social services experience – an equivalent combination of education and experience may be considered.
Preferred Qualifications: Direct and/or lived experience with homelessness, low-income populations, and/or addiction.
Reliable transportation.
