* Clatsop Behavioral Healthcare (CBH) has an immediate opening for a full-time Adult Outpatient Case Manager.
This position will provide direct services to adults who are experiencing mental health issues in office, community-based, home-based, and other outpatient settings.
The case manager works as part of a collaborative process with the therapist and adult outpatient team to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet the individual’s health and human service needs.
This position provides treatment services which include case management, care coordination, referrals, and support in accessing services, individual and group skill building, and a variety of socialization activities for clients.
Specific to case management are service planning and coordination, family and social relationship building, problem solving, skills development, and referral to additional services and support, including housing assistance, educational supports, wellness and health care, financial management and resources, empowerment and self-help, and other community resources.
This position provides occasional transportation of clients in order to facilitate access to services.
The Adult Team Case Manager will have knowledge of general mental health conditions, substance use disorders, ethical standards of the agency, administrative knowledge – including proficiency in use and management of an electronic medical record, and customer service knowledge, including compassionate and professional interpersonal interaction with both clients and co-workers.
This position will function as part of an interdisciplinary team and possesses flexibility to assist fellow team members to provide continuity and consistency of care to the agency’s clients.
Essential Responsibilities: * Maintains positive working relationships.
Works as an active and cooperative member of treatment team.
* Provides case management and care coordination services to the adult population related to identified treatment and service planning goals, in consultation with the primary therapist and treatment team.
* Assists clients in accessing additional services, community resources, and socialization opportunities as identified by treatment team and as indicated as sustaining placement and family relationships.
* Develops and maintains therapeutic relationship with client and family.
* Implements skills training utilizing individualized treatment planning goals in consultation with the primary therapist and the treatment team.
* Completes and maintains all clinical documents and client files according to compliance standards with the State of Oregon Administrative Rules, Care Oregon and Quality Assurance standards established at Clatsop Behavioral Health.
This includes, but is not limited to, the timely completion of progress notes, closings, and other Carelogic documentation, as well as monthly reporting requirements.
* Provides case management including maintaining contact with referral source(s) and other staff regarding client and significant other(s) involvement in treatment services, case-coordination with other agencies, maintaining chart notes related to external contracts, obtaining pertinent records from other treatment agencies and tracking clients involvement, follow-through, and progress in varied treatment activities.
Maintain awareness of community referral resources * Process and triage referrals between agency programs and/or with outside agencies to ensure continuity of care; * Acts as a liaison with outside agencies and specialty care settings; * Utilizes information obtained from client evaluation identify specific problems in relation to following areas: abuse/addiction; psychological/behavioral; legal; medical; family; social/relationships; educational/occupational/financial; * Facilitates or co-facilitates group services which may include: insight and awareness groups; early stage treatment groups; recovery-model groups; continuing-care support services; criminality treatment groups.
* Attends informational and clinical meetings which may include all-agency meetings, team meetings, consultation meetings with psychiatrist, staffings, intra-agency trainings, etc.
* Adheres to the Oregon Administrative Rules (OAR) and standards of care, including client confidentiality.
* Ensures implementation of case management goals from the individual service plans.
* The ability to remain flexible to the changing demands of a mental health system and the needs of the agency.
* Transports clients to facilitate client-directed services.
* Other duties as assigned.
*Education and Experience* * Registered QMHA through MHACBO or ability to register withing 30 days of hire.
* Bachelor’s degree (B.
A.
) from four-year college or university is preferred or three years related experience and/or training; or equivalent combination of education and experience.
* Have knowledge, skills, and abilities demonstrating competence in: treatment of mental illness, program policies and procedures for client case management and record keeping; and accountability for recording information in the client files assigned to them consistent with those policies and procedures.
Should be familiar with theory or concepts of prevention.
* Demonstrate effective communication skills and ability to provide culturally-competent treatment services to diverse client populations*.
* * Proficient in or ability to learn required computer applications.
* Have (or ability to obtain) and maintain a valid Oregon or Washington driver’s license and meet the agency insurance standards.
